Essay submissions will close at 11:59 PM EST on Thursday, April 2, 2020. Winners will be contacted directly and formally announced in May 2020.
✓ Are a United States citizen residing in the United States who, at time of application, have been accepted to, or are currently or will be attending an accredited, nonprofit, two- or four-year college, university, or an advanced (post-high school) vocational or technical school ("qualifying institution") as a student in the upcoming school year. Proof of admission and current enrollment will be required at the time the scholarship is awarded.
✓ Have been diagnosed with a GI condition and were under the care of a licensed healthcare professional in the US.
✓ Write a short essay, between 250-300 words, that describes the impact your diagnosed GI condition has had on your life, as well as the role a licensed healthcare professional played in helping to treat the condition.
✓ Obtain two (2) letters of recommendation that highlight your outstanding accomplishments in any school and/or community activities.
✓ A confirmation from a licensed US healthcare professional of the diagnosis of a GI condition.
How to Apply
✓ All essays must meet all entry requirements and be submitted via email to firstname.lastname@example.org. Any submissions mailed to Salix or handed in to a doctor or employee will not qualify.
✓ Please attach all essays as a PDF. Additionally, please attach two letters of recommendation and confirmation from a licensed US healthcare professional of the diagnosis of a GI condition.
✓ In the body of the email, please include your full name, address, phone number, the scholarship you are applying for (i.e., Undergraduate Scholar Awards, Graduate Scholar Awards, Working Parent’s Scholars Awards, or Single Parent’s Scholar Awards) and the name and address of your institution of higher education.
✓ Submissions must meet all entry requirements and be sent to: email@example.com by 11:59 PM EST on Thursday, April 2, 2020.
